Job Title: Registered Nurse – Home Health

Location: Duncanville, TX 75137

Shift: Day, 5×8-Hour (08:00 – 16:30)

Start Date: ASAP

Job Duration: 13-week renewable contract (will consider 26 week assignment)

Salary: $6,500-$8,000/month

We are seeking a compassionate and dedicated Registered Nurse to join our Home Health team. As a Home Health Nurse, you will provide comprehensive nursing care to adult patients in the comfort of their own homes. Your responsibilities will include conducting assessments, developing care plans, administering medications, performing treatments, and collaborating with interdisciplinary healthcare professionals to ensure the highest quality of patient care.

Responsibilities:

• Perform initial and ongoing assessments of patients’ health status in the home setting.
• Develop and implement individualized care plans based on patients’ needs and goals.
• Provide skilled nursing care, including wound care, medication administration, IV therapy, and other treatments as prescribed.
• Educate patients and their families on disease management, medication administration, and home care procedures.
• Collaborate with physicians, therapists, and other healthcare professionals to coordinate patient care.
• Monitor patients’ progress, document observations, and update care plans as necessary.
• Ensure accurate and timely documentation of all nursing interventions and patient interactions.
• Adhere to all regulatory guidelines, including HomeCare Home Base and Oasis requirements.
• Meet productivity targets and document at least 30 productivity points.

Requirements:

• Active state license as a Registered Nurse.
• Proficiency in HomeCare Home Base and Oasis documentation.
• Minimum of 2 years of experience in home health or a related specialty.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work autonomously and make sound clinical judgments.

Note: This position offers mileage reimbursement based on IRS standards. COVID-19 vaccination is required as per facility guidelines (medical/religious exemptions only). Local candidates will be considered.
