As a nurse in our team, you will be instrumental in developing and implementing individualized care plans, contributing to patient rehabilitation programs, and ensuring a safe and efficient work environment. Responsibilities:

• Conduct comprehensive nursing assessments within 8 hours of patient admission.
• Create individualized care plans based on assessments in collaboration with patients, their families, and our interdisciplinary team.
• Participate in patient rehabilitation programs, prioritizing patient education, goal achievement, and quality outcomes.
• Initiate discharge planning from the day of admission and prepare patient/family for the transition.
• Communicate crucial information regarding changes in patient status to all relevant care providers.
• Supervise and train ancillary nursing personnel, ensuring the efficient functioning of the unit.

Qualifications:

• Graduate of an accredited nursing program
• Current state licensure or in the process of obtaining
• Excellent organizational and communication skills
• Commitment to a team-oriented approach in a results-driven environment
• Prioritizes patient and employee safety
