Our client is a federally qualified, non-profit health center serving communities with clinical and outpatient locations servicing Upper NY area. They are a patient-centered healthcare center that provides primary, preventative, and behavioral health services to various patients regardless of their insurance status. They are seeking to add a Licensed Practical Nurse (LPN) to their team.
SALARY/HOURLY RATE:
$25/hr – $27/hr DOE
POSITION OVERVIEW:
The Licensed Practical Nurse (LPN) provides direct patient care under the supervision of a Registered Nurse, Regional Nurse Manager, or Medical Provider. The schedule is from Monday to Friday with rotating Saturdays and weekend flexibility.
RESPONSIBILITIES OF THE LICENSED PRACTICAL NURSE (LPN):
- Interviews patients about current problems, medications, and allergies and reviews medical records for pertinent previous and current information. Interviews and maintains patients’ medical record to include nursing observation, health history to include past and present use of medication, vital signs, vision/hearing tests, medications and treatments given, and reactions to tests i.e. PPD measurements, laboratory test results, utilizes patient identifiers, CDSS, and ensures that CVRs are completed as needed.
- Establishes and documents patient treatment regimen under supervision of the RN or MD and implements treatment.
- Assists in phlebotomy including identifying patient, checking requisition form for requested tests, and preparing equipment, patient and puncture site.
- Performs in-house labs and collects specimens for outside labs perform quality control in accordance with organizational protocols.
- Participates in patient treatment regimen, including administering medications (under the supervision of a physician and registered nurse).
- Initiates follow-up as necessary (as directed by provider) i.e. encounter, labs, close non-billables, etc.
- Performs variety of office responsibilities including answering phones and taking messages and performing triage within the scope of licensed practical nursing practice in an accurate and timely manner.
- Collects data on patients under supervision of the Registered Nurse or provider. May not triage, interpret patient clinical data, or act independently on such data.
QUALIFICATIONS FOR THE LICENSED PRACTICAL NURSE (LPN):
- Bilingual in Spanish is a plus. Working knowledge of an EMR is preferred.
- 1 – 2 years of experience in Primary care from a medical office, clinical or healthcare setting preferred.
- Minimum work-elated experience of 0 – 1 year.
- Preferred work-related experience of 2 – 4 years.
- Excellent interpersonal, communication, and telephone skills.
- Must be fully vaccinated.
EDUCATION REQUIREMENTS:
- High school or equivalent is required.
- Valid New York State LPN License.
- CPR certification.
- Infection Control certification.
